About Le Pan Health & Wellness
What is Le Pan?
Le Pan means “Hope” in the Chakfem language, spoken across several villages and communities in Chakfem District. Our initiative exists to bring hope to rural communities by improving access to quality, affordable, and sustainable healthcare delivered right within the community.
Our Story
Dr. Dimulok Mwamnut was born and raised in the rural Chakfem community of Plateau State, a region that has long suffered from a lack of infrastructure. Many villages are isolated, without roads, electricity, or health facilities.
One childhood incident remains unforgettable. Dr. Mwamnut’s uncle sustained a severe abdominal injury while hunting. With no clinic, ambulance, or accessible road, men carried him on their shoulders for kilometres across hills and streams, using leaves to cover his protruding intestines. Tragically, he died before reaching proper medical care.
This painful experience—over 60 years ago—still shapes the vision of Le Pan today.
As adults, repeated visits to the village revealed that little had changed. We continued to help deliver babies under unsafe conditions, transport sick children and pregnant women for long distances to the nearest hospital, and often pay for treatment that families could not afford.
The Ongoing Challenges
Despite modern advancements, many Chakfem communities still lack:
Portable water supply
Basic healthcare facilities
Adequate sanitation
Access to health education
Quick emergency response
As a result, preventable diseases remain the major causes of illness and death, with unacceptably high mortality rates, especially among children.
Our Response
As we approached retirement, we felt deeply compelled to act. We asked ourselves how we could bring meaningful help to our people. The answer became clear: We must give back, and We must seek partners who share our vision.
To begin, we personally donated ₦4,000,000 (part of an estimated USD $65,000) toward the construction of a small but functional clinic featuring:
A 4-bed facility
A mini operating theatre
A maternity/cleft suite
A small laboratory
Basic sanitation infrastructure
This clinic will serve as a base for our free medical outreaches throughout the year and provide essential primary care between outreach periods.
About Le Pan Initiatives
Le Pan Health & Wellness Initiatives is now registered with the Corporate Affairs Commission of Nigeria as a Non-Governmental Organisation (NGO). Our office is located at Tadaw House, 75 Rayfield Road, adjacent St. Finbarr’s Catholic Church.
Our mission remains simple yet powerful: To bring hope to underserved rural communities through accessible, affordable, and sustainable healthcare.
We envision: to empower rural communities with the knowledge, care, and support they need to live healthier, longer, and more hopeful lives.
Our Core Objectives

Provide health education, emergency stabilisation, disease prevention, and primary and secondary healthcare services within rural communities.Water & Sanitation:

Board of Trustees
Our Board of Trustees unites medical experts and community leaders dedicated to improving health and well-being in rural communities. With experience in medicine, public health, nursing, and community development, they guide Le Pan Health and Wellness Initiatives in making healthcare accessible, affordable, and impactful for children, women, the elderly, and underserved populations.

Dr. Ohunene Audu
Family and Palliative Care Physician based in Winnipeg, Canada. Trained at the University of Jos and University of Manitoba. Passionate about medical missions, and rural healthcare outreach.

Abiola Mwannut
Experienced nurse and midwife, trained at Ahmadu Bello University, Zaria. Former ICU pioneer in Jos hospitals and Peace Corps nurse. Dedicated to community medical outreach and volunteer service.

Ezekiel Pahalsom
Community leader and farmer from Wubel, Chakfem. Former professional driver with the family business. Committed to local self-help and development initiatives.

Dr. Pokop Bupwatda
Public Health Physician and Medical Superintendent of Gindiri Comprehensive Health Center. Fellow of West African College of Physicians, consultant at Jos University Teaching Hospital. Active in community health projects and NGOs.

Dr. Nantok Dami
Medical doctor and Lecturer in the Department of Community Health, Faculty of Medical Sciences, University of Jos. Holds an MPH from the University of Sheffield, UK. Focused on community health research, teaching, and service.

Dr. Dimulak D. Mwanmut
Occupational and Public Health Physician from Wubel. Former Chief Medical Officer, NLNG Company. Advocate for accessible rural healthcare, environmental sanitation, and clean water initiatives. Founder of Le Pan Health and Wellness Initiatives.

Mr. Onuche S. Audu
Mr Onuche S. Audu of the Africa Partners of the Navigators have been partnering and assisting Le Pan Health & Wellness Initiatives. He is our representative in Canada.
